Prepare for Your Appointment

Do I need a referral?

Insurance payers categorize registered dietitians as specialists. To ensure accurate billing for services regardless of the policy, Bare Nutrition, LLC requires a referral to be on file before the initial appointment. Patients are responsible for obtaining any necessary documentation for their care. If patients have physical copies of their referrals, they can upload the document to the documents tab of their patient portal. Referrals can also be faxed to 575.904.7240. schedule an appointment

Scheduling

  • Bare Nutrition uses an online medical record system.
  • Patients can request, cancel, or reschedule their appointments under the Appointments Tab.

Intake Paperwork

  • All intake Paperwork is done online via the Kalix Electronic Medical Record Portal.
  • Patients receive two emails: one to opt in to receive communications from the practice and another with paperwork.
  • Paperwork must be submitted in its entirety before patients are scheduled.

Viewing my Medical Records

  • Patients receive an email with a link and code when their medical records are ready to access.
