Prepare for Your Appointment

Insurance payers categorize registered dietitians as specialists. To ensure accurate billing for services regardless of the policy, Bare Nutrition, LLC requires a referral to be on file before the initial appointment. Patients are responsible for obtaining any necessary documentation for their care. If patients have physical copies of their referrals, they can upload the document to the documents tab of their patient portal. Referrals can also be faxed to 575.904.7240.

• Bare Nutrition uses an online medical record system.
• Patients can request, cancel, or reschedule their appointments under the Appointments Tab.

• All intake Paperwork is done online via the SimplePractice Electronic Medical Record Portal.
• Paperwork must be submitted in its entirety before patients are scheduled.

• Under the Documents/Uploads Tab, patients have access to their medical records.
• Apple Users can utilize the SimplePractice Client Portal App and Telehealth by SimplePractice App.
